AVOID frailty framework is an evidence-based framework that is developed by the Canadian Frailty Network (CFN) to help prevent and minimize the progression of frailty. The goal of this module is to equip you with evidence-based knowledge on healthy aging, screening tools that enable early identification and management of frailty.

Through the AVOID-based practice model, you will learn about frailty and its risk factors, along with tools for early identification of frailty and to support the management of frailty.
At the end of this module, you will be:
- Able to define the word Frailty and its consequences.
- Able to identify risk factors for frailty.
- Able to define and use of Frailty Index and Clinical Frailty Scale.
- Able to quantify frailty level using Clinical Frailty Scale.
- To become familiar with AVOID strategy to mitigate frailty.
- Able to understand frailty pathways and identify resources to manage frailty.
AVOID is an acronym for activity, vaccinate, optimizing meds, interaction, diet and nutrition components for healthy aging.
