PIECES (2-day) Program for Fraser Health Long-Term Care and Assisted Living - Day 2

This course is an online course.
The P.I.E.C.E.S.™ (2-day) Education Program is designed for Clinical Leaders e.g. Directors of Care, Resident Care Coordinators, Clinical Nurse Educators and front-line professional staff e.g. RNs/RPNs, LPNs, SW and other allied health professions. The participants are expected to share their knowledge and skills obtained through the P.I.E.C.E.S. Program back at the care home.

The P.I.E.C.E.S.™ framework supports Interdisciplinary TEAMS to detect, assess and understand complex physical and cognitive/mental health needs and associated behavioural changes in the older person. P.I.E.C.E.S.™ philosophy recognizes that the person and family are central to care together with clinical and system goals to address behaviours.

Day 2 Learning Goals:

Following the completion of Day 2, learners will have:

  • Shared their Practical Application experience, continuing to build on the learning and explored possibilities for moving forward.
  • Continued to apply the 3-Q Template, building on the learning from Day 1, with an increased focus on how the Team successfully mobilizes the care plan so it is monitored, maintained and modified as necessary.

    * Holistic and Person centred
    * For everyone, irrespective of age or presence of complexity
    * A shared responsibility
