Health Professional CareConnect Access Agreement

The Health Professional CareConnect Access Agreement outlines the terms and conditions which govern your access to patient personal information through CareConnect. It sets out the legal obligations that you assume as a CareConnect user, as well as PHSA’s powers with respect to managing your access. The purpose of the Access Agreement is to ensure compliance with the Freedom of Information and Protection of Privacy Act.
