Managing Patient and Family Expectations in Late Stage Illness and Active Dying

Goals of Care conversations can be difficult to navigate when the interdisciplinary team and the patient or family are not on the same page, especially in late stage illness and at the end-of-life. Our moderator and guest panel share strategies for having conversations with patients and families when the medical realities of illness do not align with their hope and expectations. Using a values-based approach and the principles of honesty and personhood, they provide us with tools and strategies to help us align our care with the patient’s values and wishes.
