RPACE Documentation of Goals of Care in VCH Paper Charts

This online module is designed to promote centralized, interdisciplinary documentation of Goals of Care where paper documentation exists in VCH. Staff will be able to identify the Advance Care Planning & Goals of Care Discussion Record as the form that captures centralized documentation of Goals of Care. They will learn how to access this form online and learn how to accurately complete an entry on this form. We will also review the opportunities to improve continuity of care for our patients through improved, centralized documentation of Goals of Care.
