Introduction to Advance Care Planning

Course Description

This course will guide learners to understand what Advance Care Planning is, how it benefits adults and those who care for them, how to engage in conversations about Advance Care Planning, and how Advance Care Planning is linked to Goals of Care and MOST conversations. 

Learning Objectives

Participants will gain and understanding of:

  • Define Advance Care Planning.
  • Identify benefits of Advance Care Planning.
  • Identify myths associated with Advance Care Planning.
  • Describe how all healthcare providers can be part of Advance Care Planning.
  • Locate National and Provincial resources to support Advance Care Planning.
  • Identify useful phrases for having Advance Care Planning conversations.
  • Identify tools for substitute consent.
  • Define the relationship between Advance Care Planning (ACP), Goals of Care (GOC) and Medical Orders for Scope of Treatment (MOST).
  • Identify where and how to document Advance Care Planning information.
