PHC Privacy and Confidentiality Undertaking for Student Placements (Form)

The PHC Privacy and Confidentiality Undertaking is your commitment that you will abide by the policies of Providence Health Care to protect the privacy and confidentiality of patient and staff information. Students are required to read the Information Privacy and Confidentiality Policy (link within the Confidentiality Undertaking) and acknowledge their understanding of their privacy obligations by accepting the Confidentiality Undertaking.
