This course provides guidance on the standardized workflows used to accurately document a Best Possible Medication History (BPMH) in the Cerner EHR.

It is highly recommended that the prerequisite course Documenting a Best Possible Medication History is completed or refreshed within one month of taking this course.

The BPMH is the cornerstone of the medication reconciliation process. All subsequent steps depend on an accurate and complete BPMH. Medication reconciliation is a critical component of delivering safe patient care and preventing adverse drug events.

Medication reconciliation is an Accreditation Canada Required Safety Practice and an Island Health priority to ensure patient safety.

BPMH documentation is in scope for physicians, nurses, pharmacists and pharmacy technicians.

IMPORTANT - PLEASE READ THE FOLLOWING:
• Before registering for this course, it is imperative that:
• You have registered for PRIME.
• Your Cerner position has the functionality to access PharmaNet using the External Rx History button. If not, connect with Clinical Informatics.

• This interactive learning experience is intended to increase BPMH documentation competency.
• Active participation is required.
• Learners will document a BPMH during the session and will receive feedback by email after the session.
• If key concepts are not demonstrated, this will result in an incomplete grade and learners will be invited to repeat the course.
• The course is offered virtually on Teams.
• Requirements to attend are a quiet space without interruptions, a computer station with two monitors, speakers and a microphone to communicate effectively with the instructor. A single monitor, tablet or smart phone cannot be utilized. A webcam is helpful, though not necessary.
