This course introduces Primary Care teams’ role in chronic disease management and provides opportunities to practice interdisciplinary assessment, education, and care planning for chronic conditions.
Through interactive learning, participants will:
Describe the role of Primary Care teams in chronic disease management.
Become familiar with self-management as a strategy to support client care.
Learn how to use clinical guidelines and VCH Practice documents to support and plan care.
Gain experience creating and modifying care plans in Profile EMR.
