Best Possible Medication History (EHR)

Welcome to the Best Possible Medication History (EHR) course! Completing this course is a requirement for health care professionals to document a Best Possible Medication History (BPMH) in Cerner.

Documenting a BPMH is the first and most important step of the medication reconciliation process. All subsequent steps depend on an accurate and complete BPMH. Medication reconciliation is a critical component of preventing adverse drug events and delivering safe patient care

Medication reconciliation is an Accreditation Canada Required Organizational Practice (ROP) and an Island Health priority.

Medication Safety highly recommends taking this course annually to maintain competency.

After successfully completing this course, the learner will be able to:

  • Explain why the medication reconciliation process is important for patient safety
  • Explain when a BPMH is documented and when medications are reconciled
  • Explain why a BPMH requires at least two reliable information sources
  • Locate relevant policy and procedure documents
  • Document an accurate BPMH
