FH Home Health – COPD Management (Online)

This COPD (Chronic Obstructive Pulmonary Disease) Management on-line module is developed for Nurses working in Home Care Nursing in order to support their care of clients living with COPD.

  • This course explains the causes of COPD, how it is diagnosed, managed, and how to treat a COPD exacerbations.
  • The Care Management Framework is used in providing care to clients living with COPD.
  • A palliative approach to care is used with these clients by a collaborating interdisciplinary team.

    • Prior to beginning the on-line modules:
  • Complete the Self-Management Support (SMS) education as part of the Care Management Education series. It is highly recommended that you also complete the Model of Accountable Care (MAC) and the Care Transitions sessions.
  • Complete the ‘initial’ section of the Self-Assessment Competency Tool which you can download from the FHpulse, Home Health, Home Health Education site under the tab ‘BreatheWELL’ . This tool will guide you in your learning needs and self-assessed competency in caring for this population group
  • Please obtain a copy of the book “Living Well with COPD” or refer to website www.livingwellwithcopd.com
  • Please download a copy of the Quick Reference Tool for Home Care Nurses: COPD Respiratory Assessment which you will find on the FHpulse, Home Health, ‘Forms and Tools’ section
