This full day in-person course provides an overview of Advance Care Planning foundational concepts and foundational skills.
Learning Objectives:
- Learn about Advance Care Planning.
- Understand consent, communication, & shared decision making.
- Review the role and responsibilities of Substitute Decision Makers (SDMs)
- Discuss legal options for clients (Representation Agreements and Advance Directives).
- Practice conversation skills using recommended tools.
- Improve documentation skills.
