This full day in-person course provides an overview of Advance Care Planning foundational concepts and foundational skills.

Learning Objectives:

  1. Learn about Advance Care Planning.
  2. Understand consent, communication, & shared decision making.
  3. Review the role and responsibilities of Substitute Decision Makers (SDMs)
  4. Discuss legal options for clients (Representation Agreements and Advance Directives).
  5. Practice conversation skills using recommended tools.
  6. Improve documentation skills.

FHA - Advance Care Planning: Foundational Skills (Session 2 of 2)


Fraser Health clinicians who support clients though the Advance Care Planning process. This includes physicians, nurse practitioners, nurses, social workers, OTs, PTs, spiritual care practitioners and others.
